Donald Kubacki Memorial Endowed Scholarship
To provide financial assistance to a student pursuing a Human Services degree with an emphasis in Addiction Studies. Scholarships to annually benefit degree-seeking students enrolled in the Institute for the Study of Drugs/Alcohol and Addictive Behaviors.
• Human Services major with concentration in Addiction Studies • 3.0 cumulative GPA • Minimum 12 credit hour enrollment • Entering, transfer, or continuing student • This scholarship is usually given to a person entering their junior or senior year. • Be degree seeking • Be recognized as an above average student • Have demonstrated commitment to being an addiction professional • Financial need, based on FAFSA will be considered, but will not be the first criteria in selection the recipient of the scholarship • An application essay that describes the student’s desire to work as an addiction treatment professional, his/her philosophy of helping and description of long term professional goals; • Two letters of recommendation supporting the student’s application; • Applicants with a history of chemical addiction must be alcohol/drug free for a minimum of two (2) years.
